Lock Haven Rehabilitation & Senior Center | Lock Haven, PA
Lock Haven Rehabilitation & Senior Center is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team. This is an excellent opportunity to work in a collaborative Skilled Nursing and Long-Term Care environment dedicated to helping residents maximize their independence and quality of life.

Qualifications
Current Pennsylvania Occupational Therapist license or eligibility for licensure
Strong clinical, communication, and documentation skills
Commitment to patient-centered care
Ability to work effectively within a multidisciplinary team
SNF/LTC experience preferred, but not required

Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled occupational therapy services focused on improving independence with daily living activities
Collaborate with physicians, nursing staff, and rehabilitation team members
Complete timely and accurate documentation
Educate residents, families, and caregivers on treatment goals and progress
Ensure compliance with facility, state, and federal regulations
